Initial Evaluation

Comprehensive first visit to understand the diagnosis, home setup, mobility needs, safety risks, and family support. You leave with clear priorities, next steps, and a recommended plan of care.

Private Physical Therapy Session

Private in-home treatment focused on mobility, gait, balance, safety, and day-to-day function. Sessions include skilled progression, home carryover, and documentation support when needed.

* Family training and home assessment are typically included for patients who would benefit from skilled physical therapy services. The $200 fee is waived for patients on caseload.

Wellness Sessions Only

Strength and conditioning is wellness-based support, not a physical therapy evaluation or treatment visit. Think personal training, but with a physical therapist designing the exercise plan and guiding you through each exercise.

Hourly wellness session for supervised strength, balance, endurance, and movement practice.

Eight wellness sessions to use within 3 months. Available Thursdays and Saturdays only.

The $120 rate covers a one-on-one physical therapy treatment session focused on your mobility, balance, gait, strength, safety, home program, and plan progression.
Yes. The initial evaluation is $150 and includes assessment, goal setting, clinical reasoning, and recommendations for the right plan of care.
A personal electrical stimulation device may be recommended when it supports carryover between visits. Your therapist will explain whether it is appropriate and how to use it safely.
Yes. Physical therapy is generally an eligible HSA/FSA expense. Check with your plan administrator if you have specific eligibility questions.
STRIVE is currently private pay. Superbills are provided for possible out-of-network reimbursement. Medicare Part B enrollment is pending.
